Using the DA Form 2404 for Operator/Crew PMCS

(2) When more than one DA Form 2404 is needed for an inspection or

service, number the pages. Print the page number at the right side of the

form's title block. If two DA Forms 2404 are used for a service, print "1

of 2" on the first page and "2 of 2" on the second page.

(3) When used as a DEF MAINT Form, operators or crews should check

the form before each dispatch. Look for faults that may affect the mission,

and faults that are overdue to be fixed. For example, look at any dates in

column d that have passed, or services that have already been pulled. Tell

the maintenance supervisor about any that are found.

(4) Maintenance supervisors or leaders should review the DEF MAINT

forms periodically; check the status of parts on order; look for any faults

that have been corrected but not closed out; and, check for any faults

overdue to be fixed. A second DA Form 2404 DEF MAINT may be kept for this

purpose.

(5) The DA Form 2404 used for PMCSs, and the DA Form 2404 DEF MAINT

on an item of equipment, are kept in the equipment record folder until

completed, closed out, or no longer needed. Mark or highlight the DEF MAINT

form to keep it from being confused with the PMCS form. Also, do not start

a DA Form 2404 DEF MAINT until there is an action to defer. A second copy

of this form may be kept, when needed, for maintenance supervisors or

leaders.

(6) Parts on order, or actions pending under anticipated not mission

capable (ANMC) conditions, may go on the DA Form 2404 DEF MAINT with a

diagonal status symbol.

d. Using the DA Form 2404 for Operator/Crew PMCS (see figure 3 on the

following page).

(1) Block 1 - Organization.

Enter the name of the unit that owns the

equipment in block 1.

(2) Block 2 - Nomenclature and Model.

The noun abbreviation and

model of the equipment go in block 2. For floating equipment, use the noun

abbreviation and hull design number.

(3) Block 3 - Registration/Serial/NSN.

Enter the serial or

registration number of the equipment in block 3.

For floating equipment,

enter the DA hull number.
